HOW TO QUALIFY
The program serves children between the ages of 0 to 13.
Current DSHS Provider 1 award letter and one of the approved listed below:
Driver’s license, or photo I.D. with current address, as listed on award letter. All documentation will be verified.
OR
Proof of Federal or State funded low income program such as:
ECAP, Head Start, WIC, Free or Reduced lunch
TWO OF THE FOLLOWING SHOWING CURRENT ADDRESS:
Driver’s License
Current Phone Bill
Current Electric Bill Post Marked Envelope – with Name, Address, and Zip code
